Licensed Therapist – Memory Care & Cognitive SupportLMFT, LCSW, LCPC, or LMHC | Flexible 1099 Role | Established On-Site CaseloadSkills, Experience, Qualifications, If you have the right match for this opportunity, then make sure to apply today.Silver Rising Therapy is seeking a compassionate, patient, and creative LCSW, LCPC, LMFT or LMHC to provide on-site emotional support and cognitive stimulation to older adults living in a dedicated memory care community in Burr Ridge.About the RoleThis is a unique clinical role working primarily with individuals experiencing dementia, advanced cognitive decline, and age- or end-of-life-related needsThis work often looks different from traditional outpatient therapy. The focus is on meeting residents where they are cognitively and emotionally and adapting your approach based on what is most meaningful and beneficial to that individual.Depending on the resident, sessions may include:Music, familiar photographs, or reminiscenceCognitive stimulation and simple activities or gamesSensory-based engagementValidation, redirection, and supportive conversationEmotional support surrounding grief, loss, aging, and life transitionsComfort and companionship for residents with advanced dementia or limited verbal communicationSome residents may be highly conversational, while others may be minimally verbal or nonverbal. Sessions are tailored to each resident and may range from shorter visits to more traditional therapy sessions.Success may look different with this population. Sometimes it's therapeutic progress; other times it's reducing anxiety, encouraging engagement, providing comfort, or simply creating a meaningful moment in someone's day.What Silver Rising Therapy ProvidesOur model is designed to offer clinicians much of the freedom and autonomy of private practice without the responsibility of building a practice from scratch.Established caseload: SRT provides referrals and clients, with a full-time caseload available at this community.Flexible scheduling: You determine your own schedule based on your availability and resident needs.One convenient location: Your clients are located within the same community, allowing you to move from resident to resident without traveling between appointments.Administrative support: We handle client eligibility, billing, and other administrative responsibilities so you can focus your time on providing excellent care.Clinical autonomy: You have the flexibility to determine how to structure your day and approach each resident based on their individual clinical needs.Training and resources: We provide training on our clinical processes, Medicare documentation, CPT coding, and Ensora EHR.Bi-weekly compensation: Clinicians are paid based on completed sessions and documentation rather than waiting for payer reimbursement.Who Thrives in This RoleBecause of the population, temperament is just as important as clinical experience. We are looking for someone who is:Exceptionally patient, compassionate, and comfortable meeting residents where they areCreative in finding ways to connect when traditional conversation isn't possibleComfortable working with significant cognitive impairment and nonverbal residentsFlexible when a resident's abilities, mood, or availability change from day to dayAble to find meaning in small moments of connection and engagementComfortable working independently while also collaborating with community staff, families, and the SRT teamPassionate about improving quality of life for older adultsPrevious experience in memory care, dementia care, hospice, palliative care, skilled nursing, assisted living, geriatrics, or end-of-life care is highly valued, but not required.Role ResponsibilitiesProvide individualized emotional support, cognitive stimulation, and therapeutic services to memory care residentsAdapt sessions to residents' cognitive abilities, communication styles, moods, and needsConduct initial evaluations and develop individualized treatment plansFind creative ways to engage residents who may not be able to participate in traditional talk therapyMaintain accurate and timely clinical documentationCollaborate with community staff and interdisciplinary care teamsCommunicate with families and POAs when clinically appropriateSupport residents navigating dementia, grief, loss, anxiety, depression, aging, and end-of-life transitionsIndependently manage your schedule and caseloadQualificationsMaster's degree in Social Work, Counseling, Psychology, or related fieldActive independent state licensure as an LCSW, LCPC, LMFT, LMHC, or equivalentClinical experience preferredSenior care, memory care, hospice, or geriatric experience preferredStrong interpersonal and communication skillsAbility to work independently and manage your own scheduleComfort adapting traditional clinical skills to residents with varying cognitive abilitiesProof of independent licensure and malpractice insuranceAbility to pass a background checkWhy Silver Rising Therapy?Silver Rising Therapy offers clinicians the opportunity to maintain independence and flexibility while still having the support and infrastructure of an established organization.
